What is An Aftercare Program, and Why is it Important?
Addiction is a disease that cannot be cured. People who struggle with substance abuse problems have either already been in recovery or are looking for a rehab facility to get the help that they need. Regardless of the stage that a person is in, one thing is for sure, the possibility for relapsing is inevitable, because “Once an addict, always an addict.”
As mentioned before, despite all the tools available for individuals who are being treated for substance use disorders like an alcohol use disorder (AUD), the probability that an individual will engage in a successful program of recovery without relapse is less than 50 percent in many cases. While this is unfortunate, the rate in which people are relapsing can be avoided with the right treatment program.
“Aftercare” is the term used to define any type of extra treatment received for substance abuse that occurs “after” receiving treatment for the first time at a rehab facility. An aftercare program is designed to help those suffering from addiction get their lives back. This is done by providing people with the right tools and resources on how to cope with life after treatment and beyond, so they can reach the goal of optimal recovery and long-term sobriety.
It is important to reiterate, that people who have completed an extended period of time at a rehab facility for a substance use disorder (SUD), are not cured. What is does mean, is that they have managed to complete initial “stages” of their recovery process, which is no doubt a significant accomplishment.

The 6 Stages of Our Program:
Stage 1: Initial Assessment
In your first days with us, you’ll meet with a clinician who will help identify your treatment goals, introduce you to the aftercare process, and address any immediate job or legal concerns.
Stage 2: Case Management Screening
Our team conducts a detailed screening to better understand any job or legal issues that may need attention during or after your treatment.
Stage 3: Client Liaison Meeting
During your first week with us, our client liaisons engage in meaningful conversations about aftercare, emphasizing the importance of following recommended levels of care upon discharge.
Stage 4: 14-Day Review
At the 14-day mark, our case management team collaborates closely with the placement team to provide personalized aftercare options tailored to your unique needs.
Stage 5: Client-Centered Decision-Making
You’ll have the freedom to choose the most suitable aftercare option based on your needs and preferences, with support from our case manager.
Stage 6: Medical Care Coordination
Our medical case manager will work with you to schedule essential medical appointments for discharge, ensuring a smooth transition, including Primary Care Physician (PCP) visits and Medication-Assisted Treatment (MAT) if required.

Remember, while you’re in residential treatment—and even while you’re in our Intensive Outpatient Program—you’ll have strong professional and peer support network. Without continuing to recover through our Aftercare program, you’ll no longer have that additional support. While you may think that you’ve gained enough knowledge to stay sober, remember that recovery is an ongoing process.

One of the best ways to stay connected with CVRC and friends in recovery is to join our Alumni program. Ongoing 12-step program meetings and sober living arrangements can help your recovery and promote camaraderie. But alumni support is important as well. Through our growing Alumni program, you can connect with others who went through the same facility as you.
